CLIENT INTAKE FORMS

MASSAGE & BODYWORK: You will be asked to complete a brief intake form online when you schedule your initial session. If you have scheduled your session with us directly, you can download the massage intake form at the link below, and bring it to your appointment. Therapists will review the intake form answers prior to your arrival. A brief chat will take place at the start of your initial session.

H.A.R.T. METHOD MAYA ABDOMINAL: Please complete the online intake form when scheduling, or download the intake form below. During the initial 90 minute session, we will spend about 10-15 minutes at the start of the session to review some additional in-depth questions regarding reasons for your visit, goals and other details regarding the H.A.R.T. Method specific process.

FIRST APPOINTMENT
You will be contacted by email to confirm your first appointment. Please arrive a few minutes early for your first appointment.

FOLLOW UP EMAIL CONSULTATIONS
Rates include follow up email consultations as needed and within reason, including any questions, follow up discussion or follow up self-care solutions.

CANCELLATION POLICY
A 24 hour cancellation notification is required. If you do not provide a cancellation notification prior to 24 hours or simply do not show up for your appointment, you will be charged the full amount for the session. If you are not able to make it or cancel within 24 hours due to an emergency or illness, there will be no charge. If possible, a phone call would still be appreciated.

HOW TO CANCEL:
You can cancel online via your confirmation or reminder email. Click the link in the email to reschedule or cancel. If you are cancelling within 24 hours, you much TEXT or CALL the number in the email. If there is no answer, please leave a voicemail.

APPROPRIATE CONDUCT:
Skyhaven Massage & Healing Arts is a professional, therapeutic establishment. There is a ZERO TOLERANCE policy for any inappropriate sexual suggestive comments, requests or behaviors. It is illegal to solicit or request any sexual services. If such an event happens, your session will be ended immediately and you will be required to pay the full amount.

PAYMENT METHODS
Payment can be completed online after you book your session OR payment can be made at the end of the appointment session. Cash, Checks and Credit Cards are accepted. All Coupons and other discounts will be honored at the time of payment. HSA Cards are also accepted, if services are in compliance with your HSA policies.

LATE ARRIVALS
If you arrive late for your appointment, you will only receive treatment for the amount of time remaining for your session. You will still be responsible for payment of the full session amount. It is always best to try to arrive a few minutes early in case of traffic or other delays.
